What this study is about

This trial is testing whether a tailored survivorship care plan, designed for patients with low health literacy, improves understanding and access to cancer care compared to a standard plan. It focuses on Black or African American men with localized or oligometastatic prostate cancer who may have difficulty understanding their treatment options and side effects.
